De acordo com estudos populacionais, a preval\u00eancia de CMD \u00e9 de cerca de 0,036 a 0,400%.<\/p>\n<figure class=\"wp-block-video\" id=\"animacao-3d-cardiomiopatia-dilatada\"><video controls><source data-src=\"https:\/\/storage.googleapis.com\/dev_wiki_voka_io_303011\/articles\/en\/acquired-heart-diseases\/dilated-cardiomyopathy\/dilated-cardiomyopathy.webm\" type=\"video\/webm\"><\/source><\/video><figcaption class=\"wp-element-caption\">Anima\u00e7\u00e3o 3D: cardiomiopatia dilatada<\/figcaption><\/figure>\n<h2 class=\"wp-block-heading\" id=\"etiologia\">Etiologia<\/h2>\n<p>A etiologia da cardiomiopatia dilatada (CMD) \u00e9 muito heterog\u00eanea e inclui causas heredit\u00e1rias (gen\u00e9ticas\/familiares) e adquiridas:<\/p>\n<h3 class=\"wp-block-heading\" id=\"causas-geneticas\">Causas gen\u00e9ticas<\/h3>\n<ul class=\"wp-block-list\">\n<li>\u00c9 herdada predominantemente de forma autoss\u00f4mica dominante.<\/li>\n<li>At\u00e9 50% dos casos podem ter origem familiar.<\/li>\n<li>Os genes principais s\u00e3o TTN, LMNA, FLNC, BAG3, DSP, RBM20, MYH7, SCN5A.<\/li>\n<li>Pode estar associada a arritmias, anormalidades de condu\u00e7\u00e3o ou um fen\u00f3tipo sobreposto (por exemplo, com evid\u00eancias de cardiomiopatia arritmog\u00eanica).<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"causas-inflamatorias-pos-miocardite\">Causas inflamat\u00f3rias (p\u00f3s-miocardite)<\/h3>\n<ul class=\"wp-block-list\">\n<li>Frequentemente secund\u00e1ria a infec\u00e7\u00f5es virais (parvov\u00edrus B19, HHV-6, adenov\u00edrus, enterov\u00edrus).<\/li>\n<li>Pode ser autoimune, associada a doen\u00e7as como l\u00fapus eritematoso sist\u00eamico, sarcoidose, artrite reumatoide, etc.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"exposicoes-toxicas\">Exposi\u00e7\u00f5es t\u00f3xicas<\/h3>\n<ul class=\"wp-block-list\">\n<li>O \u00e1lcool apresenta toxicidade mioc\u00e1rdica direta, especialmente em doses elevadas e com uso prolongado.<\/li>\n<li>Quimioterapia: antraciclinas (doxorrubicina), trastuzumab, inibidores de checkpoint imunol\u00f3gico.<\/li>\n<li>A coca\u00edna e as anfetaminas causam vasoespasmo e danos diretos ao mioc\u00e1rdio.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"disturbios-metabolicos\">Dist\u00farbios metab\u00f3licos<\/h3>\n<ul class=\"wp-block-list\">\n<li>Defici\u00eancia de tiamina, carnitina, sel\u00eanio, zinco e cobre.<\/li>\n<li>Hipotireoidismo, hipertireoidismo, diabetes mellitus, feocromocitoma.<\/li>\n<li>Doen\u00e7as de acumula\u00e7\u00e3o: hemocromatose, doen\u00e7a de Fabry, amiloidose.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"taquiarritmias\">Taquiarritmias<\/h3>\n<ul class=\"wp-block-list\">\n<li>Fibrila\u00e7\u00e3o atrial prolongada n\u00e3o tratada, taquicardia atrial ou ventricular, taquicardia parox\u00edstica.<\/li>\n<li>Pode ser revers\u00edvel com controle da frequ\u00eancia card\u00edaca\/ritmo circadiano.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"cardiomiopatia-periparto\">Cardiomiopatia periparto<\/h3>\n<p>Ocorre nos \u00faltimos meses de gravidez ou dentro de 5 meses ap\u00f3s o parto.<\/p>\n<h3 class=\"wp-block-heading\" id=\"casos-idiopaticos\">Casos idiop\u00e1ticos<\/h3>\n<p>Diagn\u00f3stico de exclus\u00e3o quando nenhuma causa secund\u00e1ria \u00e9 identificada e os testes gen\u00e9ticos n\u00e3o fornecem informa\u00e7\u00f5es relevantes.<\/p>\n<h2 class=\"wp-block-heading\" id=\"patogenese-da-cardiomiopatia-dilatada\">Patog\u00eanese da cardiomiopatia dilatada<\/h2>\n<p>Independentemente da causa, os mecanismos patog\u00eanicos s\u00e3o semelhantes: dano aos cardiomi\u00f3citos, ativa\u00e7\u00e3o da inflama\u00e7\u00e3o, remodelamento mioc\u00e1rdico e deteriora\u00e7\u00e3o progressiva da fun\u00e7\u00e3o contr\u00e1til.<\/p>\n<p><strong>Est\u00e1gios da cardiomiopatia dilatada<\/strong>:<\/p>\n<ol class=\"wp-block-list\">\n<li>Danos prim\u00e1rios aos cardiomi\u00f3citos que levam \u00e0 ativa\u00e7\u00e3o da inflama\u00e7\u00e3o:\n<ul class=\"wp-block-list\">\n<li>Gen\u00e9ticas: altera\u00e7\u00f5es na estrutura do sarc\u00f4mero, n\u00facleo, citoesqueleto, etc.;<\/li>\n<li>T\u00f3xico: ac\u00famulo de radicais livres, disfun\u00e7\u00e3o mitocondrial, dano direto \u00e0s membranas dos cardiomi\u00f3citos;<\/li>\n<li>Viral: inflama\u00e7\u00e3o cr\u00f4nica e fibrose como resultado de miocardite autoimune.<\/li>\n<\/ul>\n<\/li>\n<li>Dist\u00farbio do metabolismo intracelular do c\u00e1lcio, do metabolismo energ\u00e9tico e da apoptose dos cardiomi\u00f3citos.<\/li>\n<li>Remodela\u00e7\u00e3o mioc\u00e1rdica: afinamento da parede, dilata\u00e7\u00e3o da cavidade (principalmente do ventr\u00edculo esquerdo, menos frequentemente de ambos os ventr\u00edculos), desenvolvimento de fibrose intersticial. O objetivo do diagn\u00f3stico \u00e9 confirmar o fen\u00f3tipo cardiomiop\u00e1tico, determinar as causas e avaliar a gravidade das altera\u00e7\u00f5es nas estruturas card\u00edacas, bem como a probabilidade de desenvolvimento de efeitos adversos.<\/p>\n<h3 class=\"wp-block-heading\" id=\"metodos-instrumentais\">M\u00e9todos instrumentais<\/h3>\n<ol class=\"wp-block-list\">\n<li>A <strong>ultrassonografia card\u00edaca<\/strong> \u00e9 o principal m\u00e9todo de diagn\u00f3stico prim\u00e1rio:<\/li>\n<\/ol>\n<ul class=\"wp-block-list\">\n<li>VDFVE (volume dist\u00f3lico do final do ventr\u00edculo esquerdo) aumentado (>150\u2013180 mL ou indexado em >75 mL\/m\u00b2);<\/li>\n<li>Fra\u00e7\u00e3o de eje\u00e7\u00e3o reduzida (<45%);<\/li>\n<li>Hipocinesia global sem anormalidades regionais;<\/li>\n<li>Frequentemente: regurgita\u00e7\u00e3o mitral e tric\u00faspide, hipertens\u00e3o pulmonar, dilata\u00e7\u00e3o do ventr\u00edculo direito.<\/li>\n<\/ul>\n<ol start=\"2\" class=\"wp-block-list\">\n<li><strong><span class=\"glossary-term\" data-title=\"RM (Resson\u00e2ncia magn\u00e9tica)\" data-tooltip=\"A resson\u00e2ncia magn\u00e9tica (RM) \u00e9 uma t\u00e9cnica de imagem moderna n\u00e3o invasiva que produz imagens em camadas altamente detalhadas de \u00f3rg\u00e3os e tecidos internos. O m\u00e9todo baseia-se no fen\u00f3meno da resson\u00e2ncia magn\u00e9tica nuclear. A principal vantagem da RM \u00e9 a aus\u00eancia de radia\u00e7\u00e3o ionizante.\" data-link=\"https:\/\/wiki.dev.voka.io\/pt\/glossario\/rm\/\">Resson\u00e2ncia magn\u00e9tica<\/span> card\u00edaca com gadol\u00ednio<\/strong> (contraste tardio de gadol\u00ednio):<\/li>\n<\/ol>\n<ul class=\"wp-block-list\">\n<li>Esclarece a estrutura mioc\u00e1rdica e as caracter\u00edsticas do tecido: fibrose, edema, infiltra\u00e7\u00e3o gordurosa;<\/li>\n<li>Padr\u00e3o t\u00edpico na CMD: ac\u00famulo meso ou subepic\u00e1rdico de gadol\u00ednio na parede lateral ou septal;<\/li>\n<li>Indispens\u00e1vel em casos suspeitos de miocardite, sarcoidose e doen\u00e7as de ac\u00famulo.<\/li>\n<\/ul>\n<ol start=\"3\" class=\"wp-block-list\">\n<li><strong>Angiografia coron\u00e1ria por TC (tomografia computadorizada)\/angiografia coron\u00e1ria invasiva<\/strong>:<\/li>\n<\/ol>\n<ul class=\"wp-block-list\">\n<li>Realizada para descartar doen\u00e7a coronariana em pacientes com mais de 35 anos de idade ou na presen\u00e7a de fatores de risco;<\/li>\n<li>Obrigat\u00f3rio em casos de dor tor\u00e1cica t\u00edpica, anormalidades regionais no ultrassom card\u00edaco ou ac\u00famulo tardio de gadol\u00ednio.<\/li>\n<\/ul>\n<ol start=\"4\" class=\"wp-block-list\">\n<li><strong>Monitor Holter ou ECG (eletrocardiograma) ambulatorial<\/strong> (24\u201372 h):<\/li>\n<\/ol>\n<ul class=\"wp-block-list\">\n<li>Arritmias ventriculares (VE, TV), FA, taquicardia, pausa, bloqueio (especialmente se houver suspeita de forma mutante do gene LMNA);<\/li>\n<li>Auxilia na sele\u00e7\u00e3o de terapias e na decis\u00e3o sobre um cardioversor desfibrilador implant\u00e1vel.<\/li>\n<\/ul>\n<ol start=\"5\" class=\"wp-block-list\">\n<li><strong><span class=\"glossary-term\" data-title=\"Bi\u00f3psia\" data-tooltip=\"A bi\u00f3psia (do grego bios - vida e opsis - olhar) \u00e9 um m\u00e9todo de investiga\u00e7\u00e3o em que c\u00e9lulas ou tecidos (amostra de bi\u00f3psia) s\u00e3o retirados de um organismo para fins de diagn\u00f3stico. O material obtido \u00e9 submetido a um exame microsc\u00f3pico, na maioria das vezes histol\u00f3gico. \" data-link=\"https:\/\/wiki.dev.voka.io\/pt\/glossario\/biopsia\/\">Bi\u00f3psia<\/span> mioc\u00e1rdica<\/strong> (conforme indicado):<\/li>\n<\/ol>\n<ul class=\"wp-block-list\">\n<li>Se houver suspeita de: miocardite ativa, doen\u00e7as infiltrativas (amiloidose, sarcoidose);<\/li>\n<li>Uso limitado, requer leituras precisas e alto n\u00edvel de execu\u00e7\u00e3o.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"metodos-laboratoriais\">M\u00e9todos laboratoriais<\/h3>\n<ol class=\"wp-block-list\">\n<li><strong>BNP\/NT-proBNP:<\/strong><\/li>\n<\/ol>\n<p>\u00c9 o biomarcador mais sens\u00edvel de insufici\u00eancia card\u00edaca congestiva. Os n\u00edveis aumentam proporcionalmente ao grau de sobrecarga de volume e press\u00e3o. Valores elevados indicam descompensa\u00e7\u00e3o, valores baixos permitem excluir insufici\u00eancia card\u00edaca.<\/p>\n<ol start=\"2\" class=\"wp-block-list\">\n<li><strong>Troponinas card\u00edacas espec\u00edficas<\/strong> (I ou T):<\/li>\n<\/ol>\n<p>\u00c9 poss\u00edvel que ocorra uma eleva\u00e7\u00e3o moderada em casos de inflama\u00e7\u00e3o ativa (por exemplo, miocardite) ou distens\u00e3o mioc\u00e1rdica acentuada. Uma eleva\u00e7\u00e3o significativa e aguda exige a exclus\u00e3o de infarto do mioc\u00e1rdio.<\/p>\n<ol start=\"3\" class=\"wp-block-list\">\n<li><strong>Horm\u00f4nios da tireoide<\/strong> (TTG, T3 livre e T4):<\/li>\n<\/ol>\n<p>O hipotireoidismo pode causar disfun\u00e7\u00e3o sist\u00f3lica, enquanto o hipertireoidismo pode causar CMD induzida por taquicardia. <\/p>\n<ol start=\"4\" class=\"wp-block-list\">\n<li><strong>Glicose e hemoglobina glicada<\/strong> (HbA1c):<\/li>\n<\/ol>\n<p>O diabetes mellitus est\u00e1 associado ao desenvolvimento de cardiomiopatia diab\u00e9tica e tamb\u00e9m agrava o curso da insufici\u00eancia card\u00edaca. <\/p>\n<ol start=\"5\" class=\"wp-block-list\">\n<li><strong>Ferritina, ferro s\u00e9rico, transferrina, satura\u00e7\u00e3o de transferrina:<\/strong><\/li>\n<\/ol>\n<p>Permite a detec\u00e7\u00e3o de defici\u00eancia de ferro ou hemocromatose. Esta \u00faltima pode levar a uma cardiomiopatia secund\u00e1ria com disfun\u00e7\u00e3o ventricular esquerda progressiva.<\/p>\n<ol start=\"6\" class=\"wp-block-list\">\n<li><strong>Marcadores de inflama\u00e7\u00e3o e autoimunidade <\/strong>(anticorpos antinucleares, fator reumatoide, anticorpos anticardiolipina, etc.):<\/li>\n<\/ol>\n<p>Empregado quando se suspeita da natureza autoimune ou inflamat\u00f3ria sist\u00eamica da doen\u00e7a \u2014 l\u00fapus eritematoso sist\u00eamico, esclerodermia, miocardite, etc. <\/p>\n<ol start=\"7\" class=\"wp-block-list\">\n<li><strong>Enzima conversora de angiotensina, receptor sol\u00favel de interleucina-2 e c\u00e1lcio:<\/strong><\/li>\n<\/ol>\n<p>Usada quando h\u00e1 suspeita de sarcoidose card\u00edaca. Particularmente relevante quando combinado com anormalidades condutivas ou altera\u00e7\u00f5es infiltrativas pouco claras na resson\u00e2ncia magn\u00e9tica.<\/p>\n<ol start=\"8\" class=\"wp-block-list\">\n<li><strong>Exames de fun\u00e7\u00e3o hep\u00e1tica, creatinina, eletr\u00f3litos:<\/strong><\/li>\n<\/ol>\n<p>Realizados rotineiramente para determinar as manifesta\u00e7\u00f5es sist\u00eamicas da insufici\u00eancia card\u00edaca e avaliar a tolerabilidade da terapia.<\/p>\n<ol start=\"9\" class=\"wp-block-list\">\n<li><strong>Testagem gen\u00e9tica:<\/strong><\/li>\n<\/ol>\n<p>Indicada para hist\u00f3rico familiar de cardiomiopatia, morte s\u00fabita card\u00edaca, bloqueios, disfun\u00e7\u00e3o grave em idade jovem ou aus\u00eancia de causas secund\u00e1rias. S\u00e3o usados pain\u00e9is para genes associados com a CMD (mais comumente TTN, LMNA, BAG3, BAG3, FLNC, SCN5A, etc.).<\/p>\n<div class=\"social-banner-block\">\n<div
