{"id":666,"date":"2025-05-08T13:32:18","date_gmt":"2025-05-08T10:32:18","guid":{"rendered":"https:\/\/wiki.dev.voka.io\/diseases\/uncategorized\/pterygion\/"},"modified":"2025-09-03T12:02:03","modified_gmt":"2025-09-03T09:02:03","slug":"pterygion","status":"publish","type":"diseases_post","link":"https:\/\/wiki.dev.voka.io\/fr\/maladies\/ophtalmologie\/pterygion\/","title":{"rendered":"Pt\u00e9rygion : \u00e9tiologie, degr\u00e9s, diagnostic et traitement"},"content":{"rendered":"<p><?xml encoding=\"UTF-8\" ?><\/p>\n<p>Le pt\u00e9rygion est une pathologie du segment ant\u00e9rieur de l\u2019\u0153il caract\u00e9ris\u00e9e par une croissance progressive du tissu fibrovasculaire pt\u00e9rygo\u00efde vascularis\u00e9, du c\u00f4t\u00e9 de la conjonctive bulbaire, \u00e0 travers le limbe au-dessus de la corn\u00e9e adjacente.<\/p>\n<h2 class=\"wp-block-heading\" id=\"etiologie\">\u00c9tiologie<\/h2>\n<p>\u00c0 l\u2019heure actuelle, on estime que les principaux facteurs qui provoquent le pt\u00e9rygion sont une forte insolation, un changement dans la composition du liquide lacrymal, un d\u00e9s\u00e9quilibre des cytokines et des facteurs de croissance ainsi que des mutations du g\u00e8ne p53.<\/p>\n<h2 class=\"wp-block-heading\" id=\"anatomie\">Anatomie<\/h2>\n<p>Anatomiquement, le pt\u00e9rygion se compose de trois parties : la t\u00eate, le cou et le corps. La t\u00eate est une partie invasive de la masse triangulaire et constitue g\u00e9n\u00e9ralement une partie progressive. Le cou est la partie communicante entre le corps et la t\u00eate. Il recouvre le limbe et forme la partie la plus \u00e9troite du pt\u00e9rygion. Le corps est la partie conjonctivale la plus large du pt\u00e9rygion, avec la base dirig\u00e9e vers le coin m\u00e9dial de l\u2019\u0153il. Une opacification superficielle de la corn\u00e9e peut \u00e9galement se produire au niveau du sommet du pt\u00e9rygion (halo), aux stades pr\u00e9coce et avanc\u00e9 de la maladie.    <\/p>\n<p>La principale caract\u00e9ristique de l\u2019anatomie du pt\u00e9rygion est la pr\u00e9sence de vaisseaux dilat\u00e9s par rapport aux vaisseaux conjonctivaux environnants normaux. Le pt\u00e9rygion est assez translucide.  <\/p>\n<h2 class=\"wp-block-heading\" id=\"classification-du-pterygion\">Classification du pt\u00e9rygion<\/h2>\n<p>Selon la longueur, le pt\u00e9rygion est divis\u00e9 en 3 stades :<\/p>\n<ul class=\"wp-block-list\">\n<li>Stade I : la t\u00eate du pt\u00e9rygion n\u2019est observ\u00e9e que dans la zone limbale de la corn\u00e9e, les fonctions visuelles et la r\u00e9fraction ne sont pas alt\u00e9r\u00e9es.<\/li>\n<li>Stade II : la t\u00eate du pt\u00e9rygion est situ\u00e9e au milieu de l\u2019espace entre le limbe et la projection du bord externe de la pupille \u00e0 l\u2019\u00e9tat normal (3 mm). Un astigmatisme corn\u00e9en incorrect est observ\u00e9 juste devant la t\u00eate du pt\u00e9rygion, tandis qu\u2019un astigmatisme corn\u00e9en correct de petites valeurs est pr\u00e9sent dans la zone centrale. En r\u00e8gle g\u00e9n\u00e9rale, l\u2019acuit\u00e9 visuelle n\u2019est pas r\u00e9duite.  <\/li>\n<li>Stade III : la t\u00eate du pt\u00e9rygion est localis\u00e9e sur la corn\u00e9e, dans la projection du diam\u00e8tre de la pupille \u00e0 la lumi\u00e8re du jour (3 mm). En raison de l\u2019\u00e9paississement du m\u00e9ridien horizontal de la corn\u00e9e, l\u2019astigmatisme peut atteindre des valeurs allant jusqu\u2019\u00e0 13 dioptries. Les fonctions visuelles sont alors r\u00e9duites.<\/li>\n<\/ul>\n<p>Il existe \u00e9galement une classification bas\u00e9e sur l\u2019\u00e9valuation de la propension du pt\u00e9rygion \u00e0 cro\u00eetre de mani\u00e8re progressive :<\/p>\n<ul class=\"wp-block-list\">\n<li>1er degr\u00e9 : la membrane est translucide, atrophique, \u00e0 travers elle, les vaisseaux de l\u2019\u00e9piscl\u00e8re sont clairement visibles. Les risques de progression sont faibles. <\/li>\n<li>2e degr\u00e9 : actif. Le pt\u00e9rygion est translucide, situ\u00e9 au-dessus de la corn\u00e9e, les vaisseaux de l\u2019\u00e9piscl\u00e8re sont partiellement visibles. <\/li>\n<li>3e degr\u00e9 : tr\u00e8s actif. Le pt\u00e9rygion est de consistance charnue, il est opaque et de couleur \u00e9carlate. <\/li>\n<li>Irritation qui ne passe pas une longue p\u00e9riode ; <\/li>\n<li>Sensation de corps \u00e9tranger, de picotements et de br\u00fblures ;<\/li>\n<li>Hyperh\u00e9mie du globe oculaire ;<\/li>\n<li>Photophobie accrue ;<\/li>\n<li>Sensibilit\u00e9 au contraste r\u00e9duite en raison de la perte de transparence de la corn\u00e9e.<\/li>\n<\/ul>\n<p>Examens instrumentaux : <\/p>\n<ul class=\"wp-block-list\">\n<li>Contr\u00f4le de l\u2019acuit\u00e9 visuelle avec correction actuelle ; <\/li>\n<li>Autor\u00e9fractom\u00e9trie ; <\/li>\n<li>K\u00e9ratom\u00e9trie ;<\/li>\n<li>Tonom\u00e9trie ;<\/li>\n<li>Tomographie par coh\u00e9rence optique du segment ant\u00e9rieur de l\u2019\u0153il.<\/li>\n<\/ul>\n<p>Examen physique : biomicroscopie du segment ant\u00e9rieur \u00e0 lampe \u00e0 fente. <\/p>\n<h2 class=\"wp-block-heading\" id=\"tableau-clinique\">Tableau clinique<\/h2>\n<p>Le diagnostic de pt\u00e9rygion repose sur les manifestations cliniques suivantes :<\/p>\n<ul class=\"wp-block-list\">\n<li>Prolif\u00e9ration fibrovasculaire de la conjonctive dans la fente oculaire ouverte ;<\/li>\n<li>Propagation \u00e0 la surface de la corn\u00e9e ;<\/li>\n<li>Membrane triangulaire ou trap\u00e9zo\u00efdale ;<\/li>\n<li>Croissance du c\u00f4t\u00e9 nasal et temporal du globe oculaire ;<\/li>\n<li>Tumeur de couleur blanche \u00e0 rose en fonction de la pr\u00e9sence de vaisseaux sanguins ;<\/li>\n<li>Ligne \u00e9pith\u00e9liale pigment\u00e9e compos\u00e9e de d\u00e9p\u00f4ts de fer (ligne de Stocker) adjacente au pt\u00e9rygion, indiquant une \u00e9volution chronique. <\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\" id=\"traitement-du-pterygion\">Traitement du pt\u00e9rygion<\/h2>\n<p>Plusieurs options th\u00e9rapeutiques sont possibles pour le traitement du pt\u00e9rygion, de l\u2019approche conservatrice \u00e0 l\u2019ablation chirurgicale. <\/p>\n<p>L\u2019ablation du pt\u00e9rygion par voie chirurgicale ne doit pas \u00eatre accidentelle, car il y a un risque de r\u00e9cidive et d\u2019autres complications. L\u2019op\u00e9ration est indiqu\u00e9e si celui-ci provoque une sensation constante de g\u00eane, r\u00e9siste au traitement conservateur, assombrit l\u2019axe visuel ou entra\u00eene une diminution des fonctions visuelles due \u00e0 l\u2019astigmatisme induit, augmente de taille ou limite la mobilit\u00e9 de l\u2019\u0153il.  <\/p>\n<h3 class=\"wp-block-heading\" id=\"traitement-medicamenteux\">Traitement m\u00e9dicamenteux<\/h3>\n<p>Un pt\u00e9rygion enflamm\u00e9 peut provoquer des irritations, une sensation de corps \u00e9tranger et des larmoiements. Ces sympt\u00f4mes peuvent souvent \u00eatre soulag\u00e9s avec des gouttes riches en dexpanth\u00e9nol ou en acide hyaluronique, disponibles sans ordonnance.
