Examinons les \u00e9tapes cl\u00e9s de la pathogen\u00e8se\u00a0: <\/p>\n<h3 class=\"wp-block-heading\" id=\"occlusion-de-l-artere-coronaire\">Occlusion de l\u2019art\u00e8re coronaire <\/h3>\n<p>La rupture se produit le plus souvent en raison d\u2019une perturbation du flux sanguin dans le territoire de l\u2019art\u00e8re coronaire droite ou de la branche circonflexe. En cons\u00e9quence, une portion significative du sang retourne dans l\u2019oreillette gauche lors de la contraction du ventricule gauche.<\/p>\n<h3 class=\"wp-block-heading\" id=\"oedeme-pulmonaire-et-choc-cardiogenique\">\u0152d\u00e8me pulmonaire et choc cardiog\u00e9nique<\/h3>\n<p>Une forte augmentation de la pression auriculaire gauche entra\u00eene un \u0153d\u00e8me pulmonaire aigu. Parall\u00e8lement, la chute du d\u00e9bit cardiaque efficace conduit \u00e0 un choc cardiog\u00e9nique et \u00e0 une hypoperfusion des organes. L\u2019apparition est g\u00e9n\u00e9ralement signal\u00e9e 2 \u00e0 7\u00a0jours apr\u00e8s l\u2019infarctus.<\/p>\n<p>Les principaux sympt\u00f4mes cliniques incluent\u00a0: <\/p>\n<ol class=\"wp-block-list\">\n<li><strong>Dyspn\u00e9e<\/strong> aigu\u00eb, \u00e9voluant rapidement vers une orthopn\u00e9e et une insuffisance respiratoire.<\/li>\n<li><strong>\u0152d\u00e8me pulmonaire<\/strong> aigu, souvent accompagn\u00e9 d\u2019expectorations mousseuses ros\u00e9es.<\/li>\n<li><strong>Hypotension et tachycardie<\/strong> induites par une r\u00e9duction brutale du d\u00e9bit cardiaque efficace (choc cardiog\u00e9nique).<\/li>\n<li><strong>Signes d\u2019hypoperfusion syst\u00e9mique<\/strong> (oligurie, extr\u00e9mit\u00e9s froides, trouble de conscience, acidose m\u00e9tabolique).<\/li>\n<\/ol>\n<p>Un souffle systolique pr\u00e8s de l\u2019apex cardiaque peut \u00eatre faible ou absent malgr\u00e9 une insuffisance mitrale s\u00e9v\u00e8re en raison de l\u2019\u00e9galisation rapide de la pression entre le ventricule gauche et l\u2019oreillette.<\/p>\n<p>Avec une rupture partielle du muscle papillaire, les sympt\u00f4mes peuvent se d\u00e9velopper moins rapidement, ce qui peut parfois entra\u00eener un diagnostic tardif.<\/p>\n<h2 class=\"wp-block-heading\" id=\"diagnostic-des-ruptures-du-muscle-papillaire\">Diagnostic des ruptures du muscle papillaire<\/h2>\n<p>Nous allons approfondir les m\u00e9thodes diagnostiques les plus courantes pour la rupture post-infarctus du muscle papillaire.<\/p>\n<h3 class=\"wp-block-heading\" id=\"marqueurs-de-laboratoire\">Marqueurs de laboratoire<\/h3>\n<p>Les diagnostics de laboratoire ne confirment pas sp\u00e9cifiquement la rupture. Leur r\u00f4le est d\u2019\u00e9valuer la gravit\u00e9 de la maladie et des complications, ainsi que de faire des pr\u00e9dictions et de choisir des strat\u00e9gies. Les principaux marqueurs des diagnostics de laboratoire incluent\u00a0: <\/p>\n<ul class=\"wp-block-list\">\n<li><strong>Troponine (hs-cTn)<\/strong>. Son \u00e9l\u00e9vation confirme une l\u00e9sion aigu\u00eb du myocarde et permet d\u2019\u00e9valuer l\u2019isch\u00e9mie dynamiquement.<\/li>\n<li><strong>Lactate (lac)<\/strong>. Une augmentation (g\u00e9n\u00e9ralement > 2\u00a0mmol\/l) confirme une hypoperfusion tissulaire\/choc.<\/li>\n<li><strong>BNP\/NT-proBNP<\/strong>. L\u2019\u00e9l\u00e9vation refl\u00e8te une surcharge aigu\u00eb de pression\/volume.<\/li>\n<li><strong>Cr\u00e9atinine, ALT, AST<\/strong>. Marqueurs de dysfonctionnement organique.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"methodes-instrumentales\">M\u00e9thodes instrumentales<\/h3>\n<h4 class=\"wp-block-heading\">\u00c9chocardiogramme<\/h4>\n<p>Il s\u2019agit d\u2019une m\u00e9thode diagnostique critique et d\u00e9cisive, qui inclut\u00a0:<\/p>\n<ol class=\"wp-block-list\">\n<li><strong>\u00c9chographie transthoracique<\/strong>. M\u00e9thode de premi\u00e8re ligne, r\u00e9alis\u00e9e imm\u00e9diatement en cas de suspicion clinique.<\/li>\n<\/ol>\n<p><strong>Principaux param\u00e8tres<\/strong> pour l\u2019\u00e9valuation\u00a0:<\/p>\n<ul class=\"wp-block-list\">\n<li>Mobilit\u00e9 et int\u00e9grit\u00e9 des muscles papillaires\u00a0;<\/li>\n<li>Pr\u00e9sence d\u2019un fragment flottant dans le ventricule gauche ou l\u2019oreillette gauche\u00a0;<\/li>\n<li>Degr\u00e9 d\u2019insuffisance mitrale (Doppler couleur)\u00a0; signes positifs d\u2019hypertension pulmonaire\u00a0;<\/li>\n<li>Dimensions et fonction du ventricule gauche et de l\u2019oreillette gauche\u00a0;<\/li>\n<li>La largeur de la vena contracta et la densit\u00e9 du signal Doppler continu sont des indicateurs cl\u00e9s pour identifier une insuffisance mitrale significative\u00a0;<\/li>\n<li>L\u2019affichage spectral de la vitesse de flux de l\u2019insuffisance mitrale lors du Doppler montre souvent une forme triangulaire caract\u00e9ristique (au lieu de la forme arrondie typique pour l\u2019insuffisance mitrale chronique).<\/li>\n<\/ul>\n<ol start=\"2\" class=\"wp-block-list\">\n<li><strong>\u00c9chographie trans\u0153sophagienne<\/strong>. Elle est consid\u00e9r\u00e9e comme une partie de la pr\u00e9paration \u00e0 un traitement chirurgical urgent ou, pour certains patients, \u00e0 une intervention percutan\u00e9e\/un soutien circulatoire m\u00e9canique. Les options de traitement m\u00e9dical incluent\u00a0:<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>Soutien respiratoire<\/strong>. Ventilation non invasive (NIV) avec pression positive continue des voies a\u00e9riennes (CPAP). Dans les cas d\u2019insuffisance respiratoire s\u00e9v\u00e8re, utiliser la ventilation m\u00e9canique avec pression expiratoire positive (PEP).<\/li>\n<li><strong>Diur\u00e9tiques<\/strong> (plus souvent, les diur\u00e9tiques de l\u2019anse\u00a0: furos\u00e9mide\/toras\u00e9mide). Utilis\u00e9s pour d\u00e9charger la circulation pulmonaire.<\/li>\n<li><strong>Vasodilatateurs<\/strong> (nitroprussiate de sodium ou nitrates). Contre-indiqu\u00e9s en cas d\u2019hypotension.<\/li>\n<li><strong>Inotropes<\/strong> (dobutamine, milrinone). Recommand\u00e9s pour faible d\u00e9bit et signes d\u2019hypoperfusion.<\/li>\n<li><strong>Vasopresseurs<\/strong> (noradr\u00e9naline). Utilis\u00e9s en cas d\u2019hypotension s\u00e9v\u00e8re. Souvent utilis\u00e9s avec des inotropes.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"therapie-chirurgicale\">Th\u00e9rapie chirurgicale <\/h3>\n<ul class=\"wp-block-list\">\n<li><strong>Chirurgie d\u2019urgence<\/strong>. Prioris\u00e9 pour les patients pr\u00e9sentant une insuffisance mitrale aigu\u00eb due \u00e0 une rupture du muscle papillaire. Dans le cas d\u2019une rupture compl\u00e8te, un <strong>remplacement valvulaire mitral<\/strong> est plus fr\u00e9quemment r\u00e9alis\u00e9.<\/li>\n<li><strong>R\u00e9paration valvulaire.<\/strong> Le plus souvent r\u00e9alis\u00e9e en cas de rupture partielle, de stabilit\u00e9 relative du patient et d\u2019expertise \u00e9lev\u00e9e de l\u2019\u00e9quipe.<\/li>\n<li><strong>Pontage aorto-coronarien. <\/strong>Souvent r\u00e9alis\u00e9 en cas de l\u00e9sions significatives des art\u00e8res coronaires. Ce type d\u2019intervention chirurgicale est r\u00e9alis\u00e9e sous circulation extracorporelle \u00e0 travers une sternotomie m\u00e9diane.<\/li>\n<li>Des <strong>approches tr\u00e8s peu invasives<\/strong> sont permises dans des situations de stabilit\u00e9 relative, individuellement, lorsqu\u2019il n\u2019y a pas besoin de pontage coronaire et avec une expertise \u00e9lev\u00e9e de l\u2019\u00e9quipe.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"soutien-circulatoire-mecanique\">Soutien circulatoire m\u00e9canique<\/h3>\n<p>Pour les patients pr\u00e9sentant une insuffisance mitrale aigu\u00eb et un choc cardiog\u00e9nique, une stabilisation temporaire peut \u00eatre obtenue \u00e0 l\u2019aide de dispositifs de soutien circulatoire \u00e0 court terme.
