Le volume de la glande thyro\u00efde ne d\u00e9passe normalement pas 18\u2009cm\u00b3 chez les femmes et 25\u2009cm\u00b3 chez les hommes. Le goitre est 4 \u00e0 5 fois plus fr\u00e9quent chez les femmes que chez les hommes.<\/p>\n<p>Le goitre peut \u00eatre physiologique (\u00e0 la pubert\u00e9, pendant la grossesse et l\u2019allaitement) ou pathologique. L\u2019hypertrophie physiologique de la glande thyro\u00efde est due \u00e0 une augmentation des besoins de l\u2019organisme en hormones thyro\u00efdiennes. Elle est temporaire et ne n\u00e9cessite g\u00e9n\u00e9ralement pas de traitement.<\/p>\n<p><a l\u2019\u00e9pid\u00e9miologie, de l\u2019anatomie, ainsi que des caract\u00e9ristiques fonctionnelles ou morphologiques.<\/p>\n<h3 class=\"wp-block-heading\" id=\"par-etiologie-et-epidemiologie\"><strong>Par \u00e9tiologie et \u00e9pid\u00e9miologie <\/strong><\/h3>\n<ul class=\"wp-block-list\">\n<li><strong>L\u2019hypertrophie physiologique<\/strong> est due \u00e0 l\u2019augmentation des besoins de l\u2019organisme en hormones thyro\u00efdiennes (pubert\u00e9, grossesse, allaitement).<\/li>\n<li><strong>Le goitre pathologique<\/strong> survient \u00e0 la suite d\u2019une maladie thyro\u00efdienne.<\/li>\n<li><strong>Le goitre end\u00e9mique,<\/strong> r\u00e9v\u00e9l\u00e9 dans une large population, est caus\u00e9 par une carence en iode.<\/li>\n<li><strong>Le goitre sporadique <\/strong>appara\u00eet dans les r\u00e9gions non end\u00e9miques.<\/li>\n<li><strong>Le goitre familial (h\u00e9r\u00e9ditaire) est <\/strong>caus\u00e9 par la pr\u00e9sence d\u2019un d\u00e9faut g\u00e9n\u00e9tique dans la synth\u00e8se des hormones thyro\u00efdiennes.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" \u00e9lev\u00e9s.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Anatomie\u00a0:<\/strong><strong>goitre simple<\/strong> (au niveau du cou) ou <strong>goitre r\u00e9trosternal<\/strong> (situ\u00e9 sous la fosse jugulaire).<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"par-stade-degres-d-hypertrophie\"><strong>Par stade (degr\u00e9s d\u2019hypertrophie) <\/strong><\/h3>\n<ul class=\"wp-block-list\">\n<li><strong>Stade 0\u00a0:<\/strong> la glande thyro\u00efde n\u2019est pas hypertrophi\u00e9e (pas de goitre).<\/li>\n<li><strong>Stade I\u00a0:<\/strong> la glande thyro\u00efde est hypertrophi\u00e9e \u00e0 la palpation, le contour du cou n\u2019est pas d\u00e9form\u00e9 et la position du cou est normale.<\/li>\n<li><strong>Stade II\u00a0:<\/strong> l\u2019hypertrophie de la glande thyro\u00efde est d\u00e9tect\u00e9e \u00e0 la fois par palpation et visuellement (d\u00e9formation du contour du cou par rapport \u00e0 sa position normale).<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\" id=\"etiologie-et-pathogenese-du-goitre\"><strong>\u00c9tiologie et pathogen\u00e8se du goitre<\/strong><\/h2>\n<h3 class=\"wp-block-heading\" id=\"causes\"><strong>Causes<\/strong><\/h3>\n<p>La cause la plus fr\u00e9quente du goitre est la carence en iode.<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>Carence en iode\u00a0:<\/strong> une consommation r\u00e9duite en iode augmente le risque de goitre.<\/li>\n<li><strong>Maladies thyro\u00efdiennes\u00a0:<\/strong> le goitre peut provenir de l\u00e9sions auto-immunes, inflammatoires ou tumorales.<\/li>\n<li><strong>Anomalies g\u00e9n\u00e9tiques\u00a0:<\/strong> des mutations dans les g\u00e8nes NIS, thyroperoxidase, DUOX2 et d\u2019autres g\u00e8nes ont \u00e9t\u00e9 identifi\u00e9es.<\/li>\n<li><strong>Goitrog\u00e8nes :<\/strong> administration de m\u00e9dicaments (lithium, amiodarone) et aliments (chou, sorgho).<\/li>\n<li><strong>Autres facteurs\u00a0:<\/strong> exposition aux rayonnements ionisants, r\u00e9sistance \u00e0 l\u2019insuline, syndrome m\u00e9tabolique.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\" id=\"pathogenese\"><strong>Pathogen\u00e8se<\/strong><\/h3>\n<p><strong>Le goitre est une r\u00e9ponse adaptative<\/strong> des cellules folliculaires de la glande thyro\u00efde \u00e0 un processus qui bloque la production d\u2019hormones thyro\u00efdiennes.<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>M\u00e9canisme\u00a0:<\/strong> une carence en iode entra\u00eene une diminution de la production d\u2019hormones. En r\u00e9ponse, l\u2019hypophyse augmente la s\u00e9cr\u00e9tion de TSH.<\/li>\n<li><strong>R\u00e9sultat\u00a0:<\/strong> des taux \u00e9lev\u00e9s de TSH entra\u00eenent une stimulation des cellules folliculaires, une hyperplasie thyro\u00efdienne et la formation d\u2019un goitre.<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\" id=\"presentation-clinique-et-symptomes-du-goitre\"><strong>Pr\u00e9sentation clinique et sympt\u00f4mes du goitre<\/strong><\/h2>\n<p>La pr\u00e9sentation clinique du goitre est due \u00e0 deux syndromes\u00a0: le syndrome de compression des organes et tissus environnants d\u00fb \u00e0 l\u2019augmentation de la taille de la glande et le syndrome de dysfonctionnement endocrinien. La grande majorit\u00e9 des patients sont euthyro\u00efdiens et ne pr\u00e9sentent aucun sympt\u00f4me.<\/p>\n<h3 class=\"wp-block-heading\" id=\"syndrome-de-compression\"><strong>Syndrome de compression<\/strong><\/h3>\n<p>Comprend des sympt\u00f4mes tels qu\u2019une fi\u00e8vre persistante et une sensation de boule dans la gorge, une alt\u00e9ration de la phonation, une dysphagie, une dyspn\u00e9e croissante et une d\u00e9formation du contour du cou. \u00c9tant donn\u00e9 que le volume de la glande thyro\u00efde augmente g\u00e9n\u00e9ralement lentement, ces sympt\u00f4mes se d\u00e9veloppent progressivement et les patients ne pr\u00e9sentent aucun trouble pendant longtemps.<\/p>\n<h3 class=\"wp-block-heading\" id=\"syndrome-de-dysfonctionnement-endocrinien\"><strong>Syndrome de dysfonctionnement endocrinien <\/strong><\/h3>\n<p>Les manifestations cliniques sont dues au d\u00e9veloppement d\u2019une hypo- ou d\u2019une hyperthyro\u00efdie. La grande majorit\u00e9 des patients sont euthyro\u00efdiens et ne pr\u00e9sentent aucun sympt\u00f4me.<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>Hypothyro\u00efdie (goitre non toxique)\u00a0:<\/strong> fatigue rapide, l\u00e9thargie, apathie, somnolence, labilit\u00e9 \u00e9motionnelle, ongles et cheveux cassants, peau s\u00e8che, gonflements, prise de poids, baisse de la libido, r\u00e8gles irr\u00e9guli\u00e8res chez les femmes.<\/li>\n<li><strong>Hyperthyro\u00efdie (goitre toxique)\u00a0:<\/strong> tachycardie, arythmies, perte de poids malgr\u00e9 une augmentation de l\u2019app\u00e9tit, labilit\u00e9 \u00e9motionnelle, augmentation de la transpiration, tremblements des mains.<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\" id=\"diagnostic-et-traitement-du-goitre\"><strong>Diagnostic et traitement du goitre<\/strong><\/h2>\n<h3 class=\"wp-block-heading\" id=\"methodes-de-diagnostic\"><strong>M\u00e9thodes de diagnostic<\/strong><\/h3>\n<ol class=\"wp-block-list\">\n<li><strong>Anamn\u00e8se <\/strong><\/li>\n<\/ol>\n<p>Les faits suivants sont importants\u00a0: vivre dans des zones o\u00f9 l\u2019iode est d\u00e9ficitaire, avoir \u00e9t\u00e9 expos\u00e9 \u00e0 des rayonnements ionisants, avoir des ant\u00e9c\u00e9dents familiaux, prendre des m\u00e9dicaments qui affectent la fonction thyro\u00efdienne.<\/p>\n<ol start=\"2\" class=\"wp-block-list\">\n<li><strong>Examen et palpation <\/strong><\/li>\n<\/ol>\n<p>Ils permettent d\u2019\u00e9valuer la taille de la glande. \u00c0 l\u2019examen, les patients atteints d\u2019un goitre de stade 2 pr\u00e9sentent un contour du cou d\u00e9form\u00e9 avec une position normale du cou, et la peau est g\u00e9n\u00e9ralement de couleur normale.<\/p>\n<p>\u00c0 la palpation, la glande thyro\u00efde est hypertrophi\u00e9e, pr\u00e9sente une consistance \u00e9lastique, est indolore et mobile par rapport aux tissus environnants.<\/p>\n<p>En cas de goitre nodulaire\/multinodulaire, une ou plusieurs masses \u00e9lastiques indolores de forme ronde sont d\u00e9tect\u00e9es \u00e0 la palpation dans le tissu thyro\u00efdien.<\/p>\n<ol start=\"3\" class=\"wp-block-list\">\n<li><strong>Diagnostic en laboratoire<\/strong><\/li>\n<\/ol>\n<p>D\u00e9termination des taux d\u2019hormones thyro\u00efdiennes dans le s\u00e9rum (T3, T4, T3 libre, T4 libre, TSH, TG, TPOAb, TTHAb, TGAb).<\/p>\n<ol start=\"4\" class=\"wp-block-list\">\n<li><strong>\u00c9chographie <\/strong><\/li>\n<\/ol>\n<p>Il s\u2019agit de la principale m\u00e9thode d\u2019imagerie thyro\u00efdienne.
