{"id":6890,"date":"2026-05-04T16:12:14","date_gmt":"2026-05-04T13:12:14","guid":{"rendered":"https:\/\/wiki.dev.voka.io\/glossary\/subkutane-haematom-des-kopfes\/"},"modified":"2026-06-01T13:05:39","modified_gmt":"2026-06-01T10:05:39","slug":"subkutane-haematom-des-kopfes","status":"publish","type":"glossary_item","link":"https:\/\/wiki.dev.voka.io\/de\/glossar\/subkutane-haematom-des-kopfes\/","title":{"rendered":"Subkutanes H\u00e4matom des Kopfes"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Das subkutane H\u00e4matom des Kopfes (vom lat. <em>sub-<\/em> \u2013 unter, <em>cutis<\/em> \u2013 Haut und griech. <em>haematoma<\/em> \u2013 Blutgeschwulst) ist eine lokale, begrenzte Ansammlung von Blut, die sich in der Schicht des Fettgewebes strikt zwischen Epidermis und dem Sehnenhelm befindet. <\/p>\n\n<p class=\"wp-block-paragraph\">In der Traumatologie ist dies die h\u00e4ufigste und am wenigsten gef\u00e4hrliche Form von geschlossener Weichteilverletzung des Kopfes.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Bei einem direkten h\u00e4uslichen Schlag mit einem stumpfen Gegenstand oder einem Sturz kommt es zu einem Riss kleiner subkutaner Arterien und Venen. Das austretende Blut gelangt in die Schicht des subkutanen Fettgewebes (Schicht &#8222;C&#8220; in der Struktur der Kopfhaut).<\/p>\n\n<p class=\"wp-block-paragraph\">Das Hauptmerkmal der Pathogenese ist das Vorhandensein zahlreicher harter fibr\u00f6ser Septen (Trabekel) in dieser Schicht, die die Haut vertikal mit der Aponeurose verbinden. Diese Septen wirken wie Waben und verhindern, dass sich das austretende Blut seitlich ausbreitet. Infolgedessen sammelt sich das Blut lokal und dehnt die Gewebe, wodurch eine gespannte, sich \u00fcber die Oberfl\u00e4che erhebende Schwellung entsteht.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Bei der Untersuchung stellt der Arzt eine dichte, runde und stark schmerzhafte Schwellung fest, die sich bei Verschiebung der Haut mitbewegt. Ein H\u00e4matom erreicht niemals kritische Volumina, die systemischen Blutverlust verursachen k\u00f6nnten.<\/p>\n\n<p class=\"wp-block-paragraph\">Es gibt eine klassische diagnostische Falle, die mit diesem Zustand verbunden ist. In den ersten 24 Stunden werden die R\u00e4nder des H\u00e4matoms mit Fibrin durchtr\u00e4nkt und werden steinhart, w\u00e4hrend das Zentrum weiterhin mit weichem, fl\u00fcssigem Blut gef\u00fcllt ist. Bei Palpation einer solchen Struktur (dichter Rand an den R\u00e4ndern und Vertiefung in der Mitte) k\u00f6nnte ein unerfahrener Fachmann f\u00e4lschlicherweise einen eingedr\u00fcckten Sch\u00e4delknochenbruch vermuten. Zur Ausschlussdiagnose eines Bruches ist eine R\u00f6ntgenaufnahme immer erforderlich.
