Tonsillotomie (von lat. tonsillae<\/em> \u201eMandeln\u201c und altgriechisch \u03c4\u03ad\u03bc\u03bd\u03c9<\/em> \u201eschneide, zertrenne\u201c) ist eine teilweise chirurgische Entfernung (Verkleinerung) der Gaumenmandeln. Der wesentliche Unterschied zur Tonsillektomie besteht darin, dass nur der in den Rachenraum herausragende Teil der Mandel entfernt wird, w\u00e4hrend ihre Basis und Kapsel erhalten bleiben.<\/p>\n\n

Das Hauptziel dieses Eingriffs ist die Beseitigung einer mechanischen Behinderung der Atemwege, die durch \u00fcberm\u00e4\u00dfig vergr\u00f6\u00dferte Mandeln verursacht wird. Dabei erf\u00fcllt das teilweise erhaltene lymphatische Gewebe weiterhin seine Immunfunktion.<\/p>\n\n

Anzeigen<\/strong><\/h2>\n\n

Die Tonsillotomie ist eine Operation der Wahl, die ausschlie\u00dflich bei Obstruktionen durchgef\u00fchrt wird. Sie kommt bei chronischen Tonsilleninfektionen nicht zum Einsatz.<\/p>\n\n

Die wichtigste und eigentlich einzige Indikation ist eine Hyperplasie der Gaumenmandeln, die zu Atemst\u00f6rungen f\u00fchrt:<\/p>\n\n

    \n
  • Obstruktives Schlafapnoe-Syndrom (OSAS):<\/strong> die wichtigste Indikation, insbesondere im Kindesalter. Die Mandeln werden kleiner, wodurch sich die Atemwege erweitern und die Atmungsst\u00f6rungen im Schlaf beseitigt werden.<\/li>\n\n\n\n
  • Ausgepr\u00e4gtes Schnarchen (Ronchopathie),<\/strong> bedingt durch vergr\u00f6\u00dferte Gaumemandel.<\/li>\n\n\n\n
  • Dysphagie:<\/strong> in seltenen F\u00e4llen, wenn riesige Gaumenmandeln (sog. \u201ekissing tonsils\u201c) das Schlucken erschweren.<\/li>\n<\/ul>\n\n

    Am h\u00e4ufigsten wird der Eingriff im Kindesalter bei Tonsillenhyperplasie durchgef\u00fchrt. <\/p>\n\n

    Eine Tonsillotomie ist kontraindiziert bei rezidivierender Angina und dekompensierter chronischer Tonsillitis, da der verbleibende Teil der Tonsille weiterhin als Infektionsherd dienen kann. In diesen F\u00e4llen ist eine Tonsillektomie angezeigt.<\/p>\n\n

    Verfahrenstechnik und Vorteile<\/strong><\/h2>\n\n

    Die Operation wird immer unter Vollnarkose durchgef\u00fchrt. Das Ziel des Eingriffs ist es, den hervorstehenden Teil der Tonsille zu entfernen, ohne deren Kapsel und das darunter liegende Muskelgewebe der Rachenwand zu besch\u00e4digen. Moderne Verfahren:<\/p>\n\n

    • Coblation (Kaltplasma-Ablation):<\/strong> Goldstandard. Dabei wird das Gewebe genau und mit minimal m\u00f6glichen thermischen Verletzungen \u201eherausgesprengt\u201c.<\/li>\n\n\n\n
    • Microdebrider-Technik:<\/strong> Einsatz eines speziellen Werkzeugs (Shaver), das das Tonillengewebe abschneidet und aspiriert.<\/li>\n\n\n\n
    • Laser- und Hochfrequenztechniken.<\/strong><\/li>\n<\/ul>\n\n

      Vorteile gegen\u00fcber Tonsillektomie:<\/p>\n\n

      • Wesentlich weniger postoperative Schmerzen:<\/strong> die Rachenmuskulatur bleibt intakt.<\/li>\n\n\n\n

        Klinische Bedeutung und Prognose<\/strong><\/h2>\n\n

        Die Tonsillotomie ist ein hochwirksames und sicheres Verfahren zur Behandlung des durch eine Tonsillenhyperplasie verursachten obstruktiven Schlafapnoe-Syndroms bei Kindern. Die Wirksamkeit der teilweisen Tonsillenentfernung bei der Behandlung von Obstruktionen ist \u00e4hnlich wie bei der Tonsillektomie, jedoch weist sie ein deutlich g\u00fcnstigeres Sicherheitsprofil auf. Eventuell ist ein erneutes Wachstum (erneute Hyperplasie) des verbleibenden lymphatischen Gewebes m\u00f6glich, aber in der Praxis kommt das nicht oft vor. Die Wahl zwischen einer Tonsillotomie und einer Tonsillektomie h\u00e4ngt immer von der Hauptindikation f\u00fcr den Eingriff ab, u.\u00a0zw.
