W\u00e4hrend der Untersuchung des Ohrs (Otoskopie) erscheint das Trommelfell aufgrund des durchscheinenden Blutes bl\u00e4ulich, violett oder nahezu schwarz.<\/p>\n\n<p>Dabei handelt es sich nicht um eine eigenst\u00e4ndige Krankheit, sondern um ein wichtiges klinisches Merkmal, das meist auf eine Verletzung hinweist.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p>Die h\u00e4ufigste und schwerwiegendste Ursache eines H\u00e4matotympanons ist ein Sch\u00e4delbeinbruch, speziell ein Bruch der Schl\u00e4fenbeinpyramide. Blut aus der Sch\u00e4delh\u00f6hle tritt in die Paukenh\u00f6hle ein.<\/p>\n\n<p>Als weitere m\u00f6gliche Ursachen kommen direkte Verletzungen des Ohrs, Barotrauma (pl\u00f6tzliche Druckver\u00e4nderung), Blutgerinnungsst\u00f6rungen oder, in seltenen F\u00e4llen, Tumore des Mittelohrs in Frage.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p>Das Auftreten eines H\u00e4matotympanons, insbesondere nach einer Kopfverletzung, ist ein alarmierendes Signal und erfordert sofortige Aufmerksamkeit. Dies ist eines der klassischen Symptome einer Sch\u00e4delbasisfraktur, zusammen mit dem \u201eBrillenh\u00e4matom\u201c und dem Battle-Zeichen.<\/p>\n\n<p>Klinisch zeigt sich ein H\u00e4matotympanon als Schallleitungsschwerh\u00f6rigkeit (H\u00f6rverlust) auf der betroffenen Seite und als ein Gef\u00fchl von Druck oder Verstopfung im Ohr. Die Diagnose erfolgt durch den Arzt im Rahmen einer Ohrenspiegelung. Bei Verdacht auf eine Sch\u00e4delbasisfraktur ist eine Computertomographie (CT) des Kopfes dringend erforderlich.<\/p>\n\n<p>Die Behandlung zielt auf die zugrunde liegende Ursache ab.
